If ordering by fax or email please provide the following information: 

-State quantity of each model wanted 

-State method of payment 

If using a charge card.. 

-State type of card. 

-Name (as it appears on your chargecard) 

-Shipping Address (must be same as your charge card billing address) 

-Expiration date 

-Three digit code (normally on the rear of card) 

-State shipping preferance: UPS Brown Label (surface) or UPS Blue Label (2nd Day Air shipping)
